Inventor of Super-Toast
The best death I've ever had the honor of DMing was the death of one feral kobold samurai/martial artist/fighter/sorcerer/blade dancer (don't ask. please don't) named Hiroaki. The result of ophidian experiments (as were all too many characters in this reptilian campaign), Hiro's sanity was held by a thin tether- his daisho, especially his katana, which contained the spirit of an ancient ophidian warrior.
The party decides to destroy the sinister weapons-smuggling ring that is the Order of the Black Flame (little do they realize the organization's desires for world domination, nor have they met the slaad armies being bred in the Citadel of Black Flame). So, they break, enter, kill the Flame's nominal leader (really a front for its actual slaad lord and master) and are soon faced by a good-sized army of guards. Their leader is a nasty bruiser with a large spiked chain.
Hiroaki leads the charge, but the head guard is a tricky fighter, and entangles and snaps, Hiro's trusty katana.
So Hiro snaps, saying that if he is unable to extract his revenge, everybody within his reach will die. The party, wanting to stay in one piece, agrees. But Cid, the rather absent-minded cleric and nominal leader of the party, forgets to change the target of his spiritual hammer. Black Flame Elite goes down.
Within seconds, Cid's neck is in Hiroaki's hands, and the battle with the outside enemy is all but forgotten as the party tries to deal with the enemy within.
Fortunately, saner heads prevail. Cid manages to talk Hiroaki down, with the help of the rest of the party. Except the rogue, who's just finished dispatching a few mooks, sees the threat to Cid, and stabs Hiro right in the liver just as an arrangement has been met.
Exit Hiro, reduced to -9 hp, and with Cid (understandably) not terribly inclined to help him.
Truly a testament to the malfunctions of my group.
