The most FRUSTRATING thing?

Opponents who are able to cast "Heal." Our party was THIS CLOSE to killing a high-level rakshasa shaman in a 14th-level Oriental Adventures campaign and all of a sudden he cast... AGGGH! I can't even talk about it!!! ;)

(We never did kill that stupid shaman... he got away...) ;)

1. Scrags. Stupid things keep jumping into the water and regenerating when we try to light them up. Add that to their rend ability and they're a handful.
2. Behirs. I've never fought one and not had a character swallowed.

A favorite of mine to throw at my PCs (they always seem to run rather than fight) The 10 headed PyroHydra. Nice reach, 10 AoO and a 10 direction breath weapon to boot. Wooh.

On the Player side

Hydra. Nasty. Advanced 12 headed Pyrohydra with Improved (read 3.0) Haste cast on it. Nasty Nasty.

Bodaks.

Waaaay tougher than their CR would have you believe.

If you think having to make a "save or die" every round is tough, think about the poor guy it fixes it's gaze on...s/he has to save twice every round or die.

When these things show up, someone is toast.
Yeah, these guys are absolute horrors.... In one of our current campaigns, we sometimes have to face these damn beasts. I am playing a Dwarven Wizard with an unusual high fortitude saving throw.
In four encounters with Bodaks (lvl 12-14), he died twice now (one time due to a natural 1). And we don´t have a full-time cleric that could make us immune to it all the time...

Visages (used the conversion from these boards) and an effective but LOW will barbarian in the party :)

Undead templates added to..well anything. Currently I'm walking my players through a custom Dead Gods campaign in which Orcus/Tenebrous is rightly the Undead Prince :D. My players fear undead. From Spellstitched, True-Ghoul, Zombie, Skeleton, Vampire, etc. I've used them all. My favorite has been a True-Ghoul Half-Fiend Medusa 4th level Ranger which hunted them for 12 sessions. The session they 'killed' her has been the most fun session ever and the most rewarding as they still tell tales of that one during parties and other get togethers. The fun part is I've recently got Libris Mortis...

But most of the suggestions I've read are rightfully feared by players.

Gah, this reads like a list of what my group met on our last adventure. A lich, about 20 vampires, a ghost dragon, serval mobs of 5 or so bodaks at a time, spectres, wraiths, dread wraiths, amongst other undead. Oh, and some mindflayer and drow "ambassadors".

The lich was killed five times before we found his phylactory, but we about emptied his horde looting him each time. :)
The vampires were killed 3-6 times each as we sought out the coffins.
The ghost dragon was only twice, but very tough.

Turned our party into a lean, mean undead killing machine for shear survival. We were very glad to see that back of that place.

The most frustrating enemy is a high level Wizard/Fighter who is Improved invisible, flying, wearing a cape of nondetection, and shooting spells at a party. (maybe troll too with ring of fire resistance :D )
