Sliding tables - the floor tilts, sending tables sliding across the floor. Make a Jump check at DC 10 to hop up onto them as they run over combatants. Failure results in tables Overrunning combatants.

Hmmm... If you mean stuff in smaller scale combat of PC's versus 'monsters', here are some ideas:

The 'orc' (or whatever) you killed has a brother in the same band of enemies, who suddenly switches targets and attacks you in rage (barbarian rage?)

There was a charm (put on by the creature's mother? wife?) on the creature you just killed triggering some form of curse on you

In its dying grasp, your target grabs the weapon you just used to hit it and doesn't let go.

Uneven ground, when you fall, chance you hit your head causing dizziness maybe even fainting (appropriate penalties apply)

The site you are fighting on was once a shrine (good? evil?), the blood of the fallen arouses some spirit which may attack.

The site you are fighting on was once a shrine and the emenations of ancient prayers and magic keeps spirits within their bodies. Even though enemies are brought below 0 HP, they refuse to fall down and die but continue fighting. The only solution is to retreat to an area outside the influence of the ancient site.

Residue from ancient magic is triggered every time someone uses a spell. The effect may be to:
- enhance or reduce the effect of the spell just cast
- the spells energies trigger other magic, draining life force (HP. Str, Con?) in a radius of x yards around the caster of the spell or around the area of effect of the spell
- trigger another spell, maye some form of protective glyph
