IMO, the baddest mutha of them all ought to be undead - a lich, most likely.
 

A gargantuan vampiric celestial dire flumph, with epic levels out the wazoo.

It could be worshipped by psionic zombie lizard man ice hockey players from outer space. They could ride the flumph, travelling through space and time together to ruin campaigns throughout all the universes, planes and dimensions.

It would be a beautiful thing.

Maybe an adventuring party of epic level titans, who collectively could take down near anything...

If you're really after largest, you'd have to convert Rolemaster's Clostoph - a creature that is a plane unto itself. OTOH, it doesn't actually walk the lands - just occasionally sticks one of its heads through a rift in the mortal plane.
