Brief Explanation on Wild Shape Needed

If I am a level 7 druid and i wild shape into BEAR, BLACK and I have a consitution bonus of '2' and I have 45 hit points, what would my stats be after morphing? I know i retain my charisma, intelligence, wisdom, but I am not sure about attack damage, attack bonus etc.

1) You retain hitpoints, but gain lost hps as if rested.
2) Physical stats will change, but con will affect only saves etc not HPs.
3) BaB does not change, though str can change hit/dmg bonuses
4) You gain natural attacks of new form as well as AC modifiers
5) Items meld and do not function

It would look like Hp: 45 AC: 13 BaB +5 Claw +9 1d4+4 Full: 2 claws +9 1d4+4 and bite +4 d6+2, Saves: as druid and stats (dex +1, con +2, Wis normal), Str 19, Dex 13, Con 15, Skills and feats as character.
