Bull Rush into an IRON MAIDEN

That was a cool part of *Spoiler* Warning (select text to see) Keep on the Shadowfell.

When I ran our group through it we were still really new at the rules so I did not give them a save but it worked out as they got free and slammed the hobgoblin into it himself! Today I would allow a save to fall prone.

I'd consider it a Limited Use stunt rather than hindering terrain. I'd classify it as High damage, or Moderate damage with a rider effect (knocked prone and/or immobilized until end of next turn). The Limited Use accounts for the fact you can't really use the trick without an Iron Maiden nearby...

I'm polling my group, but I would probably not give them a save. If the Rule of Cool didn't convince them (which would make me cry), I could always point to p. 42 as SquireJames did above.

Yup. Our Cleric Commanded a Goblin into it, then moved over and shut the door.

Everyone else was like "D@MN!"

For some reason, killing them with an Axe seems OK, but an Iron Maiden? That's just harsh, man!

The way I see it, there shouldn't have been a save involved, and here's why. Using Keep on the Shadowfell as an example (which, I'm assuming, you were playing), if I'm not mistaken, simply being being (or being pushed) in the same square as the Iron Maiden doesn't cause damage to anybody- when someone is in the Iron Maiden's square, someone else has to taken an action (minor, I believe) to close said Iron Maiden, thus dealing damage. In this case, the Iron Maiden's square is no more hindering than a square in the middle of the battlefield, because it doesn't deal damage until someone takes an action to deal damage to you.

If, for example, someone Bull Rushed you onto the other side of a door into a room with a dragon, and then spent an action to close the door, would you get a save? Of course not. The Bull Rush isn't causing you damage, the action AFTER the Bull Rush is causing you damage.
