It has to do with the campaign feel.

<...> medieval European setting, with kings and plague, and filth in the street.

Or, one where elven queens ride upon fluffy dragons, and what-have-you.
I voted 3. The #1 is too hopeless for me, but I would certainly hate #10 (elven queens on fluffy dragons). Now, I do like Dragonlance but can't determine what would be the rating... #5 I guess?

Well, I don't aim for too high a level of realism with umpteen non-human intelligent races and things like magic working it's a bit crazy to try and say this is a realistic pseudo-medieval world.

I put 5 due to the wide variety of possibilities it allows for. I like realism but I do not like getting bogged down in the minute details of what happens in combat. I am not interested in finding out whether the cut from the sword leads to some infection. I am not too interested in keeping track of the price of grain from town to town. I like magic items but do not like magic item stores. I like dragons but do not like it when citizens have all seen one.

I didn't vote since I don't see how the original scale has anything to do with realism. I find Greyhawk much lower on the realism scale than Eberron because it has no internal consistency with the kind of magic and various other species inhabiting the same planet. On the whole, unless it's got a radically different magic system (and usually low-magic) from the core one, any pseudo-medieval setting tends to be incredibly unrealistic.
