Can't find a monster

I can't remember a monster I want to use. I've got a temple of a no-longer-worshipped Azlanti deity, and want some sort of last remnant monster to encounter the party. Kind of like a vestige, but not in the binder sense. I can convert any edition I need to, but I'd rather not make it up out of whole cloth.

Anybody have any suggestions as to what I'm looking for?

Easy. You want a:

Fiendish, wereboar/weresnake ghost. With blinking supernatural ability.

Make sure to add a weak spot that appears every 4 rounds.

Are you talking about a Vestige from the Planescape Dead Gods adventure for 2E? The vestige being an 'undead demon'. I want to say they were reprinted in a MM/MC, but can't seem to find them.
