Caster Level Check questions

Do I have this right ;

1st scenario:
To cast a Scroll of Heal, a 9th level cleric rolls a (d20 + 9) vs a DC of (11 + 1) = 12?

And if this fails, he has to make a Wisdom check (DC 5) to prevent a mishap (an automatic, unless a 1).

Question: If he succeeds at the WIS check, is the scroll used up?

2nd scenario:
Same cleric is casting a 1st level Divine Favor scroll prepared as a 18th level caster.
A check is needed? Now it's a DC of 19?

Yup.

Everything looks correct as you have it written. And the scroll is used even if the Wisdom check is made. That just prevents a mishap for reading the scroll so ineptly.
