When a player dies, he can create a new character as if he were a new player joining the campaign.
Whenever I introduce a new character to the campaign, that character is one level lower than the party average and is equipped based on NPC wealth. I do this so that the new character will not be able to outshine the other characters in the party immediately upon arrival.
