Clarification of Inspiring Word

Does a Warlord's use of Inspiring Word allow one to use a healing surge + 1d6 hp at first level or is it expending a healing surge to heal only 1d6 hit points

Does a Warlord's use of Inspiring Word allow one to use a healing surge + 1d6 hp at first level or is it expending a healing surge to heal only 1d6 hit points

Healing surge plus the 1d6 hps.

After all the text says: "Effect: The target can spend a healing surge and regain an
additional 1d6 hit points."

Note the "and"....not "instead".
