Commeth the Banhammer...

No Book of Exalted Deeds or Book of Vile Darkness.

Good call. One would also be smart in banning the FRCS for all the overpowered feats in there. In fact, unless you're running Forgotten Realms, banning all the FR books is probably not a bad idea.

Orcs - 'cause I want hobgoblins to get more visability. ;)

Lloth - I have Obox-ob playing a larger role in the sub-plots.

Indeed! Especially the Cheater (Chosen) of Mystra!

I'm allowing "More Than Everything" in my current Gestalt Planescape campaign: WotC, All Third-Party Stuff, and approved other stuff. Of course, it's near-Epic level.

For a less powerful campaign, probably much of what other people have said, adding Vow of Poverty and Sacred Vow of Peace (+/-) from BoED. I've also seen some powerful builds using Races of Stone feats.

...then again, I don't usually mind if the PC's are powerful; my villains and monsters just get ramped up to match.

There is a lot of material that I ban including stuff I don't own. Off the top of my head:

Core:
-Halfling Riding Dogs
-Orgosh's, Spiked Chains, etc.
-Most alchemical items
-feat: natural spell
-about a dozen spells (for current campaign)

Supplements: Most of these I don't own, but I have read in the bookstore

Book of Exalted Deeds
Book of Nine Swords
Expanded Psionics Handbook
Magic of Incarnum

Race of [x] almost the entire book except a few spells and the expanded skills

Complete Warrior: the base classes and a few PrCs, feats, and items

Other Complete Books: nearly everything except for the spellless paladin and ranger variants from Complete Champion and few of the following: PrCs, feats, spells, domains, and magic items

Players Handbook 2 except for the fighter variant, druid shapechange, some feats, and some spells.

Spell Compenedium 50-75% of the spells and several domains. Will probably become the entire book if and when I switch to Elements of Magic for the magic system.

The Not in Aquerra page lists a few things. . .though most of them are not necessarily directed to the players, necessarily.
Why is alchemical fire listed as being not in Aquerra, and then listed elsewhere as being something that exists with a different name?

Is it something that used to exist, but no longer does?

Races outside the Core books (for flavor reasons)

Frenzied Berserker prestige class (for irritation reasons)

Book of 9 Swords

Psionics

Everything else outside the Core Rules is judged on a case-by-case basis. So far, I haven't banned anything.
