Commeth the Banhammer...

Each campaign has a list of sources allowed. Everything not in is automatically out, unless the player comes with a very good reason for that specific option.

My DM hasn't banned much (if anything).

The players got together and effectively banned the Frenzied Berzerker on our side of the screen. Our DM has had us fight one, but when one of the players wanted to go into the class we got together and changed his mind.
 

Let's see, from memory:

All of the races from the PHB except humans.
Orcs, goblins, and most of the common "cannon-fodder" monsters.
All divine casters (including rangers and paladins).
Wizards.
All teleportation type spells.
Alignments.
All alignment based spells.

Now, I've added some other stuff - new races, and some alternate classes (although no divine classes), but you won't find any of the things listed above in my current campaign.
 

We used to have a banned list. Well more of a "only this stuff is allowed" list. Core, completes. Now though, go ahead, use it. You gotta know the rules. And there are only two item exceptions.
Magic Item Compendium: Amulet of Second Chances. The way we play there is no way I'm tracking all the damages from the last round.
Complete Arcane: Thought Bottle. No save games thanks.

Persistent spell (I allow using the same meta-magic more than once, if a 4x extend doesn't get your duration to 24 hrs then it shouldn't be 24 hours, end of story).
 

Orb Spells.
Dweomerkeeper.
Hulking Hurler.
Divine XXXXX (Metamagic, etc).
Persistant Spell.
Any PC that tries to use natural attacks / multiple arms / different types of attack sequences at once.
Anything I'm not personally familiar with.

Why is alchemical fire listed as being not in Aquerra, and then listed elsewhere as being something that exists with a different name?
Aquerra is a mystical place, I guess. :confused:

Is it something that used to exist, but no longer does?

Uh, just so people know it is asscociated with specific culture and does not go by the name "alchemical fire", nor is it something you can just buy or start with.
