how would the common cold be handled in dnd 4th edition?
I just curious on this accord.

Short answer: normal cold is covered by the general rule on environmental dangers (Rules Compendium, p. 178). 1 Endurance check per day. If you fail you lose a healing surge. Surges can be regained with an extended rest outside the harsh environment.

Long answer: D&D4 is not big on simulating the world. I think it is more a matter of the role you want cold to play in the story and/or in the encounter. So, normal cold could be treated in terms of...

-Pure fluff with no game effects
- A Skill check - see above.
- A Skill challenge - not only Endurance checks but also Nature check to build a igloo to shelter from cold (Rules Compendium, p. 149), Heal to fight the effects of cold...
- A disease. Use the disease template to simulate the effects of Hypothermia.
- A Hazard. Use the hazard template to create a cold themed hazard, like a chilling blizzard.
- A terrain. Create a cold-themed hazardous terrain.
- All of the above...
 


I don't see any examples in the D&D Insider Compendium. So I guess there is no published version of hypothermia as a disease.
Instead of creating one, my favorite quick solution would be to just use the rule on environmental dangers, using hard DC if the cold is really severe.
 





Common Cold Level 1 Disease

Stage 0: Target recovers
Stage 1: While affected by stage 1 target takes vulnerability 1 to cold.
Stage 2: While affected by stage 2 target takes vunerability 2 to cold. Character is affected by sneezing and coughing attacks during combat (and intense physical exertion in general).

Cough/Sneeze Attack: Any character action provokes the attack as an immediate interrupt +4 vs. Fortitude. Effect: Target takes 1 damage and is dazed, save ends. Recharge: 5-6.

Check: At the end of each extended rest, the target makes an Endurance check (if at stages 1 or 2):

Lower than DC 8, the character moves to stage 2 or remains at stage 2.
Lower than DC 12, no change.
DC 12 or better, character improves 1 stage.
 

one could have it advance to sinus infection which can be deadlly when it spreads to the brain... or Bronchial Infection


I recommend effects like reduced healing surges...
