How do we get the AC 19 on the swallow whole by the way? Normally it is 10+half natural. I could see 10+whole natural here, since it's more of an engulf, but 19 seems odd.

EDIT: I see, the regular gibbering mouther uses its normal AC, including Dex bonus. In that case, use the normal AC here too?

Yes it can.

It can also use its regular bite attacks against a swallowed opponent...

Agreed. It should have AC 20, same as the greater mouther's outside.

I propose you do it.

Once a week it may separate a small portion of itself, normally only one or two mouths and a few eyes, creating an independent creature called a gibberspawn.

Gibberspawn
A grapefruit-sized blob with a few eyes and mouths, a gibberspawn is a barely-intelligent bud of the greater gibbering mouther. It normally attaches itself to a living creature and slowly drains blood (1 hp per day) as its victim carries it around. The gibberspawn is able to give wordless suggestions to its host (similar to a suggestion spell, but only conveying emotions and simple thoughts), which it normally does in service to its monstrous parent. With the help of the gibberspawn, a greater gibbering mouther is able to control a significant territory. Gibberspawn are AC 7, 1 HD, have a movement rate of 3 and are worth 65 XP.

I suppose the first thing we should decide upon is whether we go with one or two mouths (or some random chance of either).

I'm guessing its Diminutive. Retain improved grab or change to attach? Blood drain should be based on bite/grapple rather than swallow whole.

The suggestion on host bit suggests some symbiont traits.

Weight and volume-wise it certainly should be Diminutive, but I reckon it'll be better off being oversized. Same as the SRD Rat, which is bigger than it should be going by the length & bodymass in the size table.

We'd be better off with Attach rather than Improved Grab.

Yes, I was also thinking of the Stirge when I typed that, hence the Attach. A stirge is Tiny and significantly bigger than a normal rat, though, which is why I used the latter in the size argument.
