Converting Planescape monsters

That would probably keep the AC in a reasonable range as well. If the mezzikim's deflection bonus stacked with the base creature's nat armor, it might get out of control.

Works for me.

Next: I think we should switch abilities to

Abilities: A mezzikim-ridden uses the Strength, Dexterity, and Constitution scores of the base creature and the Intelligence, Wisdom, and Charisma scores of the mezzikim.

since the possession is like magic jar and all.

Not much left except for the CR, huh? Which is going to be tricky. The mezzakim itself can't do any damage, but it's incorporeal, invisible, and likely to possess someone to fight for them.

Before we get to CR, we need to add levitation and the spike-spitting ability!
While controlling the victim, the mezzikim can levitate at will. Additionally, he increases the host's Strength by 2, to a maximum of 19. Once per turn, he can spit needles covered with poisonous bile at an opponent within 10 feet. These needles inflict 1d8 damage and the target must save vs. poison or take an additional 1d6+1 damage from the bile.
So levitate at will as SLA and a breath weapon doing piercing and poison? Or just piercing and acid? Or make it a ranged touch? Also, I'm thinking 1d8+1d6 damage seems a little low. Thoughts?

I think we might put in notes about how to end the possession. Certain spells (too bad exorcism isn't core!) including maybe dismissal or banishment would make sense. Also, the original text suggests that hp damage to the mezzikim will do the job. Since we're keeping separate hp totals, say the mezzikim leaves when it's reduced to half of its own hp?

Yeah, I'm starting to agree with you. I guess 1d8 plus 1d6 acid is reasonable enough, though I wouldn't require an extra save.
