The giant leech will be in the upcoming Tome of Horrors by Necromancer games, but I'm not aware of any other sources for a conversion of it.
 


Giant Leech: CR 2; SZ M Vermin; HD 2d8+2; hp 11; Init +1; Spd 20 ft.; AC 11; Atk +1 melee (1d6, bite); SA attach, blood drain (1d4 temp Con), disease (see below); SQ vermin, scent, salt vulnerability (1d4 per pound of salt poured on it); AL N; SV Fort +4, Ref +1, Will +0; Str 11, Dex 12, Con 12, Int —, Wis 10, Cha 2.

Skills: Hide +6, Spot +5.

Attach (Ex): A giant leech that hits with its bite attack latches onto the opponent’s body. An attached giant leech loses Dex bonus to AC and thus has an AC of 10.

Blood Drain (Ex): A giant leech drains blood for 1d4 points of temporary Constitution damage each round it remains attached. Once it has drained 4 points of Constitution, it detaches to digest its meal.

Disease (Ex): Fifty percent of all giant leeches
carry disease. Filth Fever — bite, Fortitude save (DC
12), incubation period 1d3 days; damage 1d3 temporary
Dexterity and 1d3 temporary Constitution (see “Disease” in the DMG, Chapter 3).

I think there was a giant leech in a Dungeon Magazine with a jungle adventure. I think it was the one that first introduced the Huecuva template... (Looking through the WotC site, I think it is issue #86, the adventure Rana Mor. If you have it, read through the adventure. They do not have a Monster Manual style entry, just a stat block)
