mh = Miniatures Handbook

I have distilled the rules for cover as best I can below. The rules for ranged cover and reach cover still have some questions. Let me know what you think. Also let me know if you find any other confirming or contradicting references.

Cover for ranged attack. Choose corner of your square. If any line from this corner to any corner of target’s square passes through square or border that blocks line of effect or provides cover, or through square occupied by creature, target has cover. (ph 150)

target’s square clarified/changed to be entire space (mh 95) (any references besides this?)

border that blocks line of effect clarified to not count lines that touch a corner or go down an edge as providing or blocking line of effect or cover because they do not pass through the border (mh 95)

Cover for melee attacks. Target does not have cover if all lines from all corners of one of your squares to all corners of one of target’s squares does not go through a wall. If target not adjacent to you, use rules for ranged attacks. (ph 151) (does this mean you can now pick just one corner of one of your squares and also can no longer pick single square of large creature???)

The actual rules you see in the Miniature's Handbook are for the D&D Miniatures game, which is a "dumbed down" version of D&D combat. If you're playing the roleplaying game, use the rules in the Player's Handbook, not the Mini's. :)

The monsters, classes, feats, and spells in the first third of the MiniHB are all made for D&D, I should add.

Mini Handbook

Note that the Mini Handbook did not actually change any of the rules in the PHB, just clarified what was meant (with the possible exception of ranged cover and what is meant by the target's square which is not even spoken of in the PHB that I can find, except for its glaring absence in the large creature discussion and melee cover).
