Crazy PC deaths

So we were gaming yesterday and the current DM is running bastion of lost souls module. Im a 20th level monk 1st level psychic warrior. We made it the the positive energy plane and headed toward this huge snowflake, where this half dragon half demon is suppose to dwell. There appeared to be an inactive portal guarded by several tanar'ri demons which were taken care of quick enough and a gargantuan maybe even larger sized Retriever. I am kinda am the tank of the group so I rush the spider like construct and start beating on it.
The DM asks where im attacking it. I say the neck area on its bottom side. Its turn comes and it squashes me. No save! I used a immediate action to blink failed by 3% and took 3338 points of damage. I have 260 hps. BUT....i am actually not dead yet because the cleric was able to buff me with delay death, and tons of other spells.
But this is unheard of for me: Someone taking 1000+ points of damage, even though it is possible. I was just wondering if anyone else would like to share some epic deaths that happened to their characters or near death experiences. ;)

The character I'm currently playing has been killed by a dread wraith via constitution drain, came back as a dread wraith, got destroyed by the party as the dread wraith, then Restored and Resurrected back to life to continue adventuring with said party. This is the second time in this campaign that this order of events has happened to my character(the first time was a different character, but still).

Oh, and a couple of years ago we were playing through RttToEE and while fighting a couple of fiendish tyrrannosaurs, my character got swallowed whole, then cut his way out, and before he could stand up got swallowed whole again by the same monster, cut his way out, and the next round got swallowed whole by the other fiendish tyrannosaur. He didn't die in that fight though, which is the funniest part.

Original Dark Sun campaign: My Dwarf charges, ignoring the summoned cloud interposed between them by the evil caster, relying on his Dwarven nature to keep him safe.

He charged in...and failed his dwarven magic resistance check (only 1% chance) and he failed his save vs the poison (only 1% chance), meaning he fell retching to the ground.

Judging the Dwarf to STILL be the more dangerous threat, the caster followed up with an attempt to polymorph him into a toad. Again, he failed his dwarven magic resistance check (only 1% chance), then he failed his save vs system shock (only 1% chance) and died.

As the fight continued, the dwarf's remains were within the AoE of a fireball that went off. For giggles, I rolled to see how he would have done. Yep- more failed saves.
 

Back in early 3e I was playing a Monk, around 6th level, with the Flying Kick feat and slippers that also doubled my charge dmg (as long as I jumped, and attacked unarmed). So my guy would routinely charge into combat, leap into the air, and land a punishing kick on my foe... often knocking them out cold.

I had gotten cocky. After much success, we were attacked by a (juvenile? reduced HD or something) Purple Worm. Well, naturally I charged in... and provoked... and got bitten and swallowed hole.

Naturally, the DM and all the other players lost it... laughter rang out for a good five minutes. About three rounds later, my monk had died of acid and crushing dmg... wishing he had bothered to carry a kama or something.
 

Gnome Child + Poison

I am relatively new to DnD, only played about 5 sessions and have now DMed 3 as well (low level). In the most recent of which after an encounter where one PC got knocked out and another (innocent naive half-orc barbarian who has to pass a will save after raging) feinted and had to be carried off back to the camp by the NPC orc. The other decided to loot the little gnome tents alone where a gnome child surprised attacked him, he was on 2hp and lucky that the child could only do 1 damage, but following this attack he took poison damage from a viper that had attacked him at the end of the encounter and fell, dying in the tent. He managed to stabilize himself on -8 though. :). We are all noobs but having so much fun!
 

Not epic, but...

PC Fighter beaten to death by goblin children wielding small baseball bats (like the ones you get on Bat Day at your local ball park).

The heck?
The only way it can deal that much is if it was falling. Falling object damage would work, but not by anything else.
Not even dragons can (Crush attacks)
Crush (Ex)

This special attack allows a flying or jumping dragon of at least Huge size to land on opponents as a standard action, using its whole body to crush them. Crush attacks are effective only against opponents three or more size categories smaller than the dragon (though it can attempt normal overrun or grapple attacks against larger opponents).
A crush attack affects as many creatures as can fit under the dragon’s body. Creatures in the affected area must succeed on a Reflex save (DC equal to that of the dragon’s breath weapon) or be pinned, automatically taking bludgeoning damage during the next round unless the dragon moves off them. If the dragon chooses to maintain the pin, treat it as a normal grapple attack. Pinned opponents take damage from the crush each round if they don’t escape.
A crush attack deals the indicated damage plus 1½ times the dragon’s Strength bonus (round down).
