Critical failure optional rules and a critical miss table

So I know critical misses aren't for everyone, but for those interested I wrote up some quick optional rules for 5th edition D&D and created a critical miss table. Take a look and let me know what you think.

If it doesn't include heart attacks, strokes and spontaneous human(oid) combustion, a fumble system is too tame for my tastes.

If it does and they come up 1% of the time, it's too harsh.

For me, a good fumble system is a wonderful thing to have, but it's a very difficult thing to achieve. I do like your "sunder the planar barriers" fumbles, though. I'll have to add 'em to my system next time I revise.
 

My problems with this are as follows:
  • weapons are usually strong enough to hold up to attacks by other weapons, I don't think just dropping one on a crit failure of 8 would be enough to break it.
  • Magical items cannot be broken except under certain circumstances. (Unless they're consumable.)

I don't really think of it as just dropping it. I think of it as swinging your weapon against the ground or the wall and it breaks.
