What are some of your favorite cursed magic items for screwing with the PCs? Here's something I just thought of:

Chickenwing Emblem

This pin appears to be a Safewing emblem (Magic Item Compenium p 131, 1 use triggers feather fall if wearer falls 10 feet). In reality, however, when it triggers it turns the wearer into a chicken for 1d6 hours.

The only one I've used so far was a Bracelet of Nine Lives.

When worn it can't be removed. If your HP goes below 20% of your total, the Bracelet automatically heals you to full HP. But each time it does this, it tightens around your wrist.

It functions 9 times and gets tighter each time it's activated. On it's 9th activation, it severs the hand from the wrist.

The party Half-Giant lost his hand this way. He had an Abishai blacksmith create him removable wrist weapons. Sorta like the BAT from G.I. Joe :p
 

I made up one, named the Loadstone Ring.

It looked like a ring of feather fall but instead - the wearer automatically failed Ref saves vs pits and falls.

At the time, I let just a regular remove curse handle its removal, but in a higher level game I think I would have added a casting of feather fall as well for good measure.

I had a homebrew item in my old campaign called the Helm of Blind Faith. It allowed the wearer to gain the benefits of Incense of Meditation, but the visor also slammed down over his face, with no eyeholes. The helm couldn't be removed by normal means.

This was in 2E - in 3E, there's too many ways around being blind for me to hand this item out...
