After pondering the question for a few days now, that was the scariest thing I could think of.

I suppose an Invisible Fiendish Rust Monster followed by the Invisible Fiendish Stirge Swarm would be even worse...

Thinking back on it, Stirges must be among the critters that terrify my players the most. I ran an adventure relatively early in my last campaign where some Stirges nearly caused a TPK when first encountered. Later, two of the PC's went scouting alone and nearly got killed by the same group of Stirges again.

Much later in the campaign when the party was 10th level or so, I had them attacked by Stirges again. Without pausing for an instant, the party Wizard cast a Lightning Ball that engulfed the whole group, inflicting considerable damage on just about everybody (except the Rogue with Evasion) for the sake of killing the Stirges before they could inflict any Con Drain. It was pretty funny.
 

LMAO Yeah it's fun watching the looks on player's faces when they realize its Stirges that they are about to encounter. With them being invisible, the characters would still hear the buzzing getting louder and louder, but would be unable to identify the source for sure until it is way too late. I wonder what the CR of that would be...
 

Specifically the Gibbering Mouther was always particularly abhorent.....

.....slimes, oozes, jellies and fungi in general.
 


Pit Fiends, but this is due to the last pit fiend BBEG I used. Gave him fiendish quickening and deeper darkness at will with Quicken Spell-like Ability. The PCs found themselves in shadowy illumination with the silhouette off a giant devil that could teleport at will as a free action. The party wizard got bite and died of poison a turn later, the fighter was power word stunned when he thought his HP were fine, a meteor swarm rained down on them, and a few rounds later found the pit fiend retreated with half the party dead. :]

Dread wraiths, anything that can spring attack from inside the walls is a whole lotta pain waiting to happen.

and oh yeah, Elephants... I don't know why or how, but Elephants freak my players out...
