I've used a scenario involving terror attacks that result in a economic and social collapse and a full "grid down" technological failure. The result is pretty low on the mutants and radiation zones, etc, but has a general Mad Max meets The Postman feel.

Add ninjas and I'm in.

I cast my vote for the above, plus biological attacks that turn people into mutant whatevers.

All of the above, Demonic invasion while a meteor shower pounds the planet after aliens destroyed the moon to cover their drop capsules fought by roving angels of vengeance and population-wide spontaneous human mutation powered by the return of magic and gratuitous use of NBC warfare countered by fiendish disease agents.

What if Earth was invaded by the dragons from Reign of Fire... and their Githyanki allies?

Take Urban Arcana, have aliens invade, humans fight back with nuclear stikes, and the aliens respond by dropping an asteroid on them.

Other - Plague. Not a deliberate biological weapon, but some viral nasty sweeping out of Asia or Africa, leaving death in its wake. Those it does not kill it cripples.

With enough death the infrastructure collapses, causing even more casualties.

Yeah, I kind of like the Plague approach, myself. I just started reading The Stand yesterday, and I have a pretty big flu bug, so it's doubly scary.

A huge plague wipes out 90% of the population, and then cue in crazy gun-wielding psychos...

But, truth be told, for an RPG, I think you need to have some element of the supernatural or supertechnological, simply to add variety to encounters and to diversify PC roles. So, I would go for the "RIFTS approach", and make the apocalypse a sort of "Psychic explosion".

My own post-apoc game went this way, with many animals becoming mutated by some strange virus that "awakened innate psychic ability", a la Akira. It's a lot of fun... our group had a blast trying to avoid a giant Purple Worm while driving about the ruins of Vancouver on dirt bikes.
