d20-ifying Sanity Checks

I love d20 Cthulhu, but find the % roll Sanity checks really clunky in my d20 games. I was thinking about the following changes:

Sanity bonus: A character's initial sanity bonus is equal to his or her Wisdom score -7.

Sanity Check: 1d20 + sanity bonus, DC 15. Natural 1 always fails, Natural 20 always succeeds.

Sanity Damage Conversion.

The following table converts Sanity damage from % damage to d20 damage:

d20 Sanity damage expressed like 1/1d10 is determined by dividing 1 by the results of a 1d10 roll and rounding down.

There's a little pdf called d20 Madness by Bruce Baugh on rpgnow.com that does all of the work of converting CoC style Sanity into a native d20 environment. I highly recommend it.

Interesting. Is this a conversion of the CoC system (I would assume as made Open in Unearthed Arcana) or a parallel madness system (aka OGL horror)?

Blood and Blades: The Profiler's Guide to Slashers, condenses the Unearthed Arcana sanity rules with the OGL Horror sanity rules to create a cohesive system that meshes the two. Might not be what you're looking for, but I like it. :)
 

I'm personally considering the Corruption system from True20 as a Madness replacement. It's not quite the same, but I think it fits pretty well.

It's a conversion of the CoC system point for point.

Sadly, the only thing missing from the CoC system is some kind of fear or horror check, though. More and more I'm leaning more towards that kind of mechanic. But I do really like the way you can see Sanity dwindle during the course of a session.

Fear Effects by 12 to Midnight.

Since I'm the president of the latter, I hope you don't mind if I pimp my product. With Fear Effects, there's no sanity score to track and the Horror table offers 100 different outcomes based on the severity of the horror check. Because it also offers several rule varients, we see it as a toolkit for GMs to create a customized sanity system to fit their particular group's needs.
