If you're addressing them in person, as opposed to just refering to them, then there's always some nice dehumanizing options.
"boy" - "You, boy, fetch me my shield"
or the always popular name that is not theirs, perhaps even not the right gender. Insist on calling all of your male cohorts 'Nancy' for example.

You could also go with Mini-(insert the PC's name).
One of my players seemed to like pissing off other players by simply calling their cohorts/followers/familiars Gary. Not sure why that always got under the other players' skins.
