i actually just played with a Dm who had a very basic, yet fully viable way of doing loot, and i felt like an idiot for not having come up with it first.
anyone ever play world of warcraft? remember the loot tokens? well there you have it.
"among the treasure hoard, you find a heavily damaged (roll for chain, leather, cloth, plate etc. + second roll for the slot which it fills) with an indistinguishable mark of a diety."
Players must then take this "token" to an altar, a church, or just spend an hour or so praying to their diety. if their diety (DM) approves of their request, the item is granted to them.
You assign this item a level, and voila. happy players, unbroken gameplay, just make sure to incorporate somehow that the item must be of a certain rarity or lower.
Example (how i would do it)
Among the ruins, you find heavily tarnished, level 14 Plate bracers. On the bottom side of the plate you notice that there is a small imprint of a diety's symbol.
(players rolls checks)
upon further inspection, it looks as though the imprint was made on top of a previous one, and that one atop one before. there is no telling how many times these bracers have been blessed/worn
