Be careful with called shots. Because then every fight will be "I target his aorta".

I was in one game where we used a house rule for called shots. If you call your target and are trying to do something "fancy", like shoot out the dragon's eye, you roll to attack. If you roll a natural 20 and confirm the critical hit, you deal no damage but instead do the "fancy" thing you wanted.

grogg They can't target the enemy's aorta in every fight because most enemies don't have exposed hearts and most adventurers don't have the knowledge of anatomy required to find it.

But I am aware that every fight becomes "I target the enemy's head with my attack", my solution is that if you aim and miss (due to the higher AC) the enemy can either choose to try to do the same to you or their first roll is a natural 20, so if they hit they crit and if they miss they hit anyway  (as do any enemies that attack them in that round)

James Good Idea and I would allow it but the grapple rules are complex enough without modification (and my group never use them for some reason)

Treat the hart as a seperate creature 4 size categories smaller and cover for which they have to ready an attack to be able to hit it.
