Fighting in the same square

I have a few questions about combat taking place in the same square.

First of all, is a a 5ft step move into another square always considered like the move to make a bull rush, or can it just be considered a 5foot step?

Also, can you and an ally fight in one square at the same time? If so, would you suffer any penalties (such as losing your dex bonus). Would an enemy attacking have a chance to hit either guy? Could two guys flank both guys at the same time, or would flanking be impossible.

And what if two guys fought each other in the same square, not grappling. Would both lose their dex bonuses, or would it be a normal fight?

I'll give you what I think are the right answers.

A - You mean into another occupied square? No, it's not always a bull rush. But it will provoke an AoO.

Q - Also, can you and an ally fight in one square at the same time?

A - No.

A - No, because you can't do it.

Q - Would an enemy attacking have a chance to hit either guy?

A - That one I'm not sure. I believe that he can choose who he hits, unless they're grappling. But since you can't fight in the same square, doesn't much matter.

Q - Could two guys flank both guys at the same time, or would flanking be impossible.

A - Again, no, because you can't do that.

Q - And what if two guys fought each other in the same square, not grappling. Would both lose their dex bonuses, or would it be a normal fight?

A - Not sure they can do that, unless one is small or tiny. But if they're not grappling (again, not sure if that's possible), they wouldn't lose their Dex bonus.

That's my thinking, but someone correct me if I'm wrong.

Allied, no. Enemy, yes.

I don't if the rules address this. I run it as a) lose Dex bonus; b) -2 to attack rolls; c) -2 to Reflex saves. I would make it a 25% chance that the wrong guy is hit. Lastly, no flanking since you have an ally between you at all times (because there is no facing in 3e).

It has to be a grapple fighting with light weapons. If the loser can't be moved into a free square (like bull rush), then he falls prone. In turn, one of them would be fighting from a prone position.

It is not a 5-foot step, actually it is not even a legal move, so there you have you can not move into somebodys square (except if you are Tiny sized or smaller).

As above it is not possible to be in the same square.

Again, not possible.
