I designed a more forgotten-realms oriented reincarnate table for our campaign:
01-12: Dwarf (01-05: arctic dwarf; 06-40: gold dwarf; 41-50: gray dwarf (duergar); 51-90: shield dwarf; 91-95: urdunnir; 96-100: wild dwarf)
13-24: Elf (01-05: aquatic elf; 06-10: avariel; 11-20: drow elf; 21-45: moon elf; 45-70: sun elf; 71-75: wild elf; 76-100: wood elf)
25-36: Gnome (01-10: deep gnome (svirfneblin); 11-25: forest gnome; 26-100: rock gnome)
37-40: Goblinoid (01-15: bugbear; 16-20: dekanter goblin; 21-60: goblin; 61-100: hobgoblin)
41-49: Half Elf (01-80: half elf; 81-85: half aquatic elf; 86-100: half drow elf)
50-57: Half Orc
58-69: Halfling (01-60: lightfoot halfling; 61-70: ghostwise halfling; 71-100: strongheart halfling)
70-84: Human (01-95: human; 96-100: deep imaskari)

100 Roll again and add template (01-35: half celestial; 36-65: half dragon; 66-100: half fiend)
We don't use level loss for reincarnate (level loss sucks, and throws a wrench into party balance), and we adjust all attribute scores, not just physical attributes. I also allow players to completely redesign their characters after a reincarnate (your dwarf fighter is now an elf wizard, etc.).
