Ghouls and other effects of first hit ?

So if a ghoul hits you are you immediately immobilized, or do you get a save right then at the point of attack? Or do you have to wait till your done with your first immobilized turn to see if you can make your saving throw?

Enemy turn: WHACK! Suffer any effects of the attack.
Start of your Turn: Suffer ongoing damage
Your Turn: Take any actions you may have remaining
End of your Turn: Roll your save if entitled

NOTE: Some attacks have "follow up attacks". Like if one makes a :bmelee: with a poisoned weapon, there might be a Follow Up Attack against the victim's fort defense done by the poison. Not all status effects do this however. Many are just riders on the attack. Get hit, get screwed.

You have to suffer one turn immobilized before attempting to save. Unless, of course, something happens before then that grants you a saving throw, like a friendly cleric using Sacred Flame.
