Gnomes - Where will they appear first?

Monster Manual is certain. They also stated, at GenCon, that they would appear in a book soon after the 3 main books were released. We know that WotC's definition of "soon" doesn't apply to the PHB II (based on this years February announcement that 4E won't be released "soon").

Of the books we know about, the FR setting seems most likely. I just don't see gnomes tied to the arcane or melee book. Indeed, that was one of the reasons I thought the half-orc was more likely (a good fit for the melee book).

Nah. The Spellplague will probably wipe out all gnomes, along with their deities. After all, they seem determined to make everything in the Realms conform to 4e changes, and that means that the races and classes that are cut from the PHB will be cut from the Realms.

I'd say Swiss Guidebook because of the Gnomes of Zürich. :p

The Germany Guidebook will feature Bayern, Pfälzer, Ostfriesten, Hamburger, and, of course, Saarländer. Said Saarländer will necessitate a late release, because they need to reintroduce stats for divine beings before they can properly portray those. :cool:

In Eberron, yes; in FR, not so much (they're called the Forgotten Folk for a reason). Anyway, they're likely going to detail gnomes as PCs in the setting book (like they did with shifters and changelings and warforged in 3e Eberron).

But what if you run a Dragonlance campaign and don't want to buy the FR campaign book just to get gnome stats?
