Grappling - I think this is easy?

A monk can use Flurry of Blows while grappling, but the extra attack from the flurry has to be an unarmed strike (at a -4 because he's grappling), not another grapple check. The 3.5 Flurry of Blows language states that the extra attack can only be used for an unarmed strike or an attack with a monk special weapon. A grapple check is neither of those.

Clarification - a monk can use Flurry of Blows while grappling, but every attack in the Flurry must be an unarmed strike (or attack with a Light monk weapon) - no grapple checks at all.

I can't make three attacks with a longsword and then an extra unarmed strike with Flurry of Blows; I can't make three grapple checks and then an extra unarmed strike with Flurry of Blows either.

Another question about grappling: I (and other players I know) seem to think that you have a 50% chance to hit the wrong guy when you attack (at least, with ranged weapons) into the grapple.

Note that it's not specified as "50% chance", but rather "determine randomly".

If there are four people involved in the grapple, you have a chance to hit any of the four.

If you have a Small creature grappling a Large creature, the DM might decided that there's some weighting on the random determination - still random, but perhaps a 75% chance the Large creature will be hit. It doesn't specify equal likelihood, so the DM's discretion is involved.

Yea, you're right. But most of the time when it turned up now, it was medium against medium, so it was 50%.
