I think that's what lots of people (including myself) don't think about. We're trained as roleplayers not to think about the acutal pain and agony our characters face. Hacked down to -9 hp is no worse than a 1 hp scratch. Wearing a 2 lb. leather jerkin is no less uncomfortable than 45 lb steel plate body armor. Heck, the only thing that bothers most of us is the movement restriction! Yeah, there's tons of reasons why characters would wear even chainmail as little as possible, but the reality of that doesn't surface in make-believe games a whole lot unless the GM enforces it.
