Does a weasel with a 14 con get +2 hp or +1? (It only has 1/2 a HD.)

Add it all up for one HD and divide by 2.

No, you'd add in the whole modifier. Examples from the MM (I'm looking at 3.0) -- Sprite (Grig), Monstrous Scorpion (Tiny).
