It does not look that powerful. It starts off as a +1 sword that lets you crit on a 19/20. I would change this to something about extending the critical range by one so that fighters that already crit on 19/20 can now crit on 18 as well.

The deal extra radiant damage but you take half in necrotic power is something I do not see being used that much, especially at higher levels when you do 3d10 extra. I can also see some people spamming this be getting necrotic resistance.

You could start the weapon off at +0 and go to +2 if you plan on giving it out by level 5 or such. I would think about giving it a rest or daily power as well to make it more unique. I tend to give out things such as reroll the last attack or gain an extra attack if you crit.
 

So the theme is "sacrifice your life to be more effective". You have focused on damage, but I don't know if that is core; a Paladin weapon could also protect.

Damage is a bit boring of a sacrifice. How about max HP?

[*] +1 longsword
[*] When you hit you can choose to deal +1d8 radiant. If you do, your max HP are lowered by 1. When this happens, the gem on the pommel turns red.

Max HP lost this way may only be recovered by sacrificing a HD (regsining 1 HP per HD) at the end of a long rest, and no more than half of your HD at the end of the rest.

As an action, a creature may touch the red gem and say "sacrifice" in any language. They lose up to 1/2 of their max HP, and for every 2 lost the wielder regains 1. If the wielder has 0 max HP, the all of the creatures max HP can instead be drained. HP are drained until either the wielder has no max HP lost, or the max amount is drained from the touching creature. They regain their max HP by sacrificng HD at the end of a long rest in the same way you could.

When this effect causes you to have 0 max HP but you still have temporary HP, you start making death saving throws but cannot stabalize. Natural 20s remove 1 failure.

Upgrade: +2/up to 2d8 at 1 HP per.

When you sacrifice HD at the end of a long rest you may roll one of them to regain that many max HP lost.

The Sacrifice of Love: When a creature within 30' of you takes damage, as a reaction you can sacrifice to prevent all of it. Your max HP are reduced by the damage prevented, and you gain an equal amount of temporary HP.

Upgrade: +3/3d8 at 1 HP per.

When you sacrifice HD to regain max HP at the end of a long rest, you regain your con bonus (min 0) max HP per HD sacrificed in addition.

The Sacrifice of Death: When you strike with this weapon, you may make your hit a critical hit and maximize all damage dice. If you do so, your max HP are reduced by half the damage you deal and you gain an equal amount of temporary HP. If this reduces you to 0 max HP and you die from it, you cannot be resurrected by any means. If someone else sacrifices for you, the same applies to them.
