I have had training in such matters, there are numerous tricks to getting the advantage in such a situation. A big problem is that you dont have the 3 points of balance firmly placed, your opponent already has one of your points offset by default.
I think that the rule about grappling with one appendage and still able to do other things would apply nicely, -20.
Even against someone with lots of training a person with a modicrum of natural ability or talent can escape fairly readily, generally also taking the other person down.
