How do you handle multiclassing?

I use the multiclassing rules in the PHB. I find it helps define racial differences as for example, almost all dwarves have a level or two of fighter, regardless of which class is highest. Of course, none of the players in any of my groups have wanted to have more than 2 base classes yet, so it hasn't really been an issue.

I haven't used any sort of training requirement, though I suppose if I used training rules for leveling, I would probably do so.

It depends on the class, but generally I require either it occurred pre-start of campaign, or PC gets special training in play. I generally don't allow multiclassing to Sorcerer, and Wizard would take years of training, not just Lidda peeking over Mialee's shoulder when she's reading her spell book in the morning! (*ugh*)
