I've oganized mine based on the usualy chapters in an RPG book, with each chapter (races, spells, feats, lands, ect...) being a seperate Word file. Every few months I update a binder that i have with the new chapters, the binder also has space at the end where my handwritten notes are stored before moving to the computer.

Well, my Players Guide to Cydra is the major resource. It's a spiral bound double sided document, divided by tabs and printed on different colored paper for each section. It's divided into:

Cydra Overview
Races
Base Classes
Feats
Equipment
Prestige Classes
Religion
Spells
Epic

Most of my maps are on larger paper, though a small regional overview map is in the PGtC. I have tons of setting info, most of it rules-outdated but still fine, spread throoughout a number of different binders of varying thickness, as well as masses of unorganized papers. I have sign in sheets from every session I've run in the current campaign world, as well as all the scratch paper I've used (really). Notes are scrawled everywhere. I also have a collection of dead pcs (the Cydran Body Bag).

Edit: Oh, there's also a ton of campaign art done by myself and various of my players over the years. :)

One Hanspring Visor plus PalmWiki. Nothing like having the ability to jump between topics via double-tap. It's not great for a straight-through read but it stores all the key data points I need to jog my memory.
