Been many years since a PC died IMC. Killed many early on in my DMing career, but have improved over 25 years. ;-)

Lots of well developed characters, roll playing, and I don't make it easy. and the real fear that I will kill them if they play badly. really I will. I mean it. don't push me!

My players are really attached to the characters and the story line is important and built with them, so it works for us. Not right or wrong, I think PC deaths would be expected and common in a 'tactical' game - but significant quantity of PC deaths don't make for a good sweeping epic.

Blood thirsty GM or stupid Players?

In the two decades that I have been playing I have seen about a dozen PCs die.

Some PCs are killed for story line, while others are off cause the Player has grown bored with them, and once in a while it just happens.

A GM that kills as often as you mentioned is just out for blood, and needs to be the one that makes up all those characters. I know I wouldn't bother- about the third character in ten sessions I would be gone from the group.
