Yes, but that assumes a connection (the Core assumes quite a bit).

As is, I'm for the demi-plane/prison-plane idea as well. I've got similar planes in the works.
 



Planar warding spell.

This is the spell I have used on multiple occasions by the mercykillers and harmonium for this specific purpose. Without the password, creatures can not pass the ward to enter that prime world. This way, until they do the necessary research (i.e. your aforementioned quest), each attempt to return them to their world will richochet them back to a random plane of your choosing. :)

What level is Planar Warding? 50th? I can't imagine a spell that can lock an entire plane of existence being cast by anything other than a god...

As for Gate, I don't believe you need any sort of connection between the planes in question. Nowhere in the PH or Manual of the Planes does it mention Gate not working when trying to create a portal to independent demi-planes, etc., even though they do take the time out to talk about how specific PH spells work in a planar setting (in MoTP). The catch is that you need to know the plane in question, so you can't Gate to a demi-plane you've never heard of. ...except we're trying to keep PCs from getting back to the Prime rather than leaving it, so they definitely know the plane they want to gate back to.

'sides, Gate requires a very high-level caster, and by then the DM will probably want them back on the Prime anyway, if not sooner by one of the color pools.

This actually could be my best option.

So how do you go about destroying portals exactly?:D

I figure if the try to go in the etheral plane or astral plane and it find it by traveling there I can just mention how dangerous it is and kinda diverge them from doing that.
 

a tricky option

Do they have to KNOW that they are exiled? What if they ended up on a plane of the abyss that looked (at first) exactly like their homeworld/plane, except the BBEG is not there (they might think that they killed HIM in whatever explosion send them to this plane). Then they find out that some/a lot/all(!) of their "friends" and "allies" and "relatives" have "become" evil. Eventually they would realize they are on an evil plane, but if done right, the deception might last long enough for your purposes. and of course you could combine this idea with one or more of the above ideas.
 

What I do is have it as a separate cosmology, even though it has similar planes. Then, instead of calling them planes, call them dimensions. That's what I'm doing with my next campaign site.
