Assuming you mean the divination spell, it is inherently not 100% accurate and the spell description makes it clear that you only get a small piece of information. As an example, you might ask "will the BBEG attack the town?" and get (up to a 90% chance of being accurate) an answer that the BBEG is marching his troops within the week - and that's it. Not how many, not how they are equipped, not what gate they are battering down or what sneaky backdoor they will use.
If you mean divination spells in general, your question is phrased such that it leads me to believe you are referring to scrying rather than the divination spell itself. Per the description of the scrying subschool, lead sheeting or magic protection blocks a scrying spell, and the target can also potentially save vs. the spell.
