First one to come to my mind is "Insert PC from another campaign here."

That's what I thought of first, as I recall Final Fantasy 1 had "Sir Edrick" dead and buried. Sir Edrick is from Dragon Warrior 1. Similar lines of thought.
 



My personal favorite:
"Here lies Lester Moore,
Four Slugs from a .44.
No Les,
No More"


And I personally came up with one for a halfling (who drank a very strong liquor called "Tup" and passed out) in our homebrew campaign this past weekend:

"Poor Garret,
Drank some Tup,
Went under the table,
and never came up."

Who died defending his right-of-way.
His course was clear, his will was strong,
But he's just as dead as if he'd been wrong.

The all-time best epitaph was from a great movie. See if you can name it:
"Died tragically saving his family from a sinking destroyed battleship."
