D&D 5E I feel all of the modules are following a theme... kind of... (potential spoilers)

What will be in the spring adventure? A duchess? A baron? Maybe a maharaja.

The Maze of the Twilight Duchess (Labyrinth workname...)

It's basically boils down to two things, I think:

-cults are convenient, as others wrote above
-because these mega-campaigns go into mid-high levels, naturally the upper echelons of power, therefore the main adversaries would be some kind of royalty/nobility because of medieval-ish generic high fantasy conventions in naming.

I do agree that there is a recurring royalty theme, and cultists do show up in them. As far as where this goes next, if we do see Undermountain, perhaps it would link in with the Lords of Waterdeep?

A good 'enemy' to throw against players, especially bloodthirsty or capable ones, are organizations. So yeah, they use cults a lot, and royalty, who would also have a wide array of underlings at their disposal. It is a good way to prevent characters from completely destroying a threatening powerful enemy all at once with lucky rolls or clever thinking.

That ONE evil wizard you had against the players? Well they caught up to him and disposed of him in quick fashion, much to your surprise an chagrin. Now what? If you have a circle of evil wizards though, there can always be another enemy to stand in the way, until such time as it feels right to end the adventure.

I mean sure you can make up reasons for not being able to find or kill the solitary bad guy, and really there is nothing stopping you from having a head of an organization or cult die to the players. The organization behind them just ensures you have a ready to tap pool of like minded villains.

I think a lot of others offered some good reasons and thoughts as well.

So for me it will never be just Bob from accounting, but the whole accounting department!

Prince of the Apocalypse
Curse of Strahd (Strahd is a count, and lives in a castle)
Rage of Demons (had to deal with demon princes, dukes, dutchesses, etc...)

Nah, while the US may have thrown off the rule of kings, they didn't actively kill too many of them.

For actual killing of royalty, one nation stands head and shoulders (well, neck and shoulders) above the others. Clearly, adventurers are French.
