Idea from a monk player

In my current campaign, the player who plays a monk got an idea, he wants to have for his unarmed attack, the same kind of magic you can apply to melee magical weapons (flaming, bane, impact, etc.) Those special attacks would come from permenant magicals tatoos or something like it.

First, is there balance issues here ?

Second, how to incorporate this in the Realms, who could "make" such tatoos, etc..

There are magic items that let your unarmed attacks have work as magic weapon.
Amulet of Mighty Fists
I think there are others as well

IIRC Arcana Unearthed has a feat that lets you enchant your fists like a weapon.
Hands as Weapons
 

You could allow a feat that the character could use to be able to 'enchant' his fists. The bonus he could get would be +1 per 4 character levels. The effect should probably have a minimum duration of one day. He could select Frost one day and then change it the next to Flaming if he wanted to.
 

Any spell that can be cast on a weapon can be cast on the monks fists anyway (it says so in the 3.5e monk description), so Greater Magic Weapon and any flaming/shocking/frosting/thundering that you allow from third part stuff can also be cast on their fists (along with psionic powers such as energy weapon, dissolving weapon etc).

It might be worth looking into the psionic tattoos rules http://wizards.com/d20/files/v35/PsionicItems.rtf for some ideas.

Take the Hands As Weapons feat from Monte Cook's Arcana Unearthed. IIRC, it has no prerequisites. It allows the character's fists to be enhanced as though they were weapons, so for instance you can get a permanent +1 enhancement bonus on unarmed strikes if someone with the Craft Magic Arms & Armor feat uses it on you, spending the normal amount of gold and time and XP to enhance your fists. I think each fist counts as a separate weapon though, be aware. And remember that you have to get a permanent +1 enhancement bonus before you can start adding special qualities like Flaming, Frost, Disruption, or Speed.

Have you any idea how amazing the existence of hands are? I mean, YOU try designing something that spectacular. Brilliant guys at MIT have been trying for years and their best still doesn't match the craftsmanship your hands have. I mean... the precision, the speed, the fluidity, the versitility, the sensitivity... it's unbelievable, really. I'd say your hands are more than masterwork, they're like epic craftsmanship or something.
 

Hands as Weapons does require a ceremony costing 100 gp per character level (when you take it).

Hands are quite amazing. I'd say more, but it'd be against the rules.
