Well, I guess if you beg...
It was part of the long standing game I run. In fact, it was the climax to the groups latest adventure: the group had been fighting a fanatical fire-based cult and had finally come to their lair which was, obviously, a volcano.
The leader of said cult (the BBEG), was a high level advanced noble salamander cleric with a number of feats taken from savage species - most notably the Multigrab feats and the Fling Enemy feat.
As everyone knows, epic fights against major villains in a volcano -must- take place on ledges, precipices, or rocky outcroppings that hang over the magma reservoir below.
(Doesn't take a genius to figure this out...)
The salamander hit the NPC with his tail attack, grappled him easily and then, while fighting the rest of the party with its longspear, flung the NPC over the side thus causing 20d6 of falling damage (they were very high up). On the NPC's turn, he took the 20d6 for lava immersion, drank the potion of
fly he had and started to ascend.
(I'd talk about the 10d6 damage a character takes the round after immersion but he was healed by then by the party cleric so it doesn't count)
He lived partly because I rolled low damage and party because of high hit points (he had been untouched up until that point).
