I vote unfair, based solely on the DC of the save.

If we assume a 9th level spell for sake of argument, then the DC would be 10 + 9 + 4 = 23. A 1st level Monk with a 14 CON will make this save on a 19 (so will a 2nd level Monk with a 12 CON). So we're looking at some kind of Epic trap.

The only way I would consider this fair would be if the PCs knew they were adventuring in a dungeon way, way out of their league, and were willingly facing extreme risks. But nothing in the writeup of the question leads me to believe this is the case.

Putting a lever there is like putting in a magic deck of cards.

Players have to pull the lever/draw the card(s). It's almost mandatory.

It's like a flash grenade that says Pick Me Up, you know they're going to.

Only way this could be more unfair is if it was a pressure trap or proximity trap.


Reminds me of my old Geometer. Glyph traps everywhere, and the DM ruled that my glyph-radar's range was less than the symbol's range, or that it was around the corner, out of sight, so I had to walk into the area to get to search for it. :\ so I never found any of them.
Until after they went off, of course.
 

I voted Other. Fair or Unfair is sort of besides the point. That sounded utterly BORING. Dungeons with traps like that frackin' suck... :(
