Islands of Peril and Lost Civilizations on a Sea Voyage....

Something I ran a long time ago -

The PCs landed on an island that was inhabited by vampire Yuan-ti, and a Coutl that had been battling them for a very long time, who was in hiding because he was outnumbered.
That one is fun.
I always wanted to do King Kong.
Also, how about an alternate world - bunch of our time soldiers on an island with machine guns, mines, etc, and the heroes have to take them out.
Make them guns not work off the island or keep the ammo low for balance.

Finally, the island that is not an island, but a colossal sea turtle.
You land on it, and it decides to leave, taking the away team underwater in it's wake, maybe even the ship itself.

More later,

An island which is used as a prison island, where criminals are exiled by a nearby country. Lots of potential politics and battles there when a non-military ship is sighted coming in!

Theres always the small isle with seven gnomish cast-aways...

Isle of the Dead - the island is tome after tome.
The Isle of the Dead is a library? ;)

(Incidentally, I didn't realize that the primary definition of tome was one of the books in a work of several volumes; the secondary definition is a book, especially a large or scholarly one.)

Black Sails over Freeport is supposed to have several mystical islands.
That does sound like the solution to DM_Matt's problem. Green Ronin's on-line store describes Black Sails Over Freeport:

Freeport's in crisis, as war breaks out on the high seas and orcs riot in the streets. A map promises the biggest haul of booty in history, but nothing is as it seems. Buried with that treasure is a terrifying evil Freeport thought banished forever. Black Sails Over Freeport, the first mega-adventure for the award-winning pirate city, is filled with enough swashbuckling challenges to test the mettle of any band of heroes. Its 256 pages are packed with action, intrigue, and danger, delivered with the style and professionalism you've come to expect from Green Ronin. Black Sails are on the horizon. Do you have what it takes to face them?
Their designer spotlight gives some more detailed info. (I don't think it quite qualifies for "spoiler" tags.)

The main action takes place on the Islands of the Damned--a mythical archipelago that appears every century and a half and hides a god's ransom in treasure. (Sort of a cross between Atlantis and Brigadoon, with a little Bermuda Triangle thrown in just because.) Each island is controlled by a different villain, and each holds a different piece of the puzzle the players will need to unlock the treasure hoard.

We made sure to give each island a distinct mood, cast of characters and set of challenges. In essence, every island is its own mini-module, as different from the rest as, say, "Expedition to the Barrier Peaks" is different from "Tomb of Horrors."

That means all sorts of possibilities for the players. They'll face classic dungeon crawls, beat-the-clock races to safety, armies of undead, giant apes, flying skulls--a great mix of hack-and-slash, problem-solving and role-playing. And, of course, at the end of it all is a huge stash of treasure. You can't spend it when you're dead!

There's also plenty in the module for diehard Freeport fans. If the Islands are the centerpiece of the adventure, Freeport is the framework--it's where you start, and where you finish (in the biggest free-for-all the city has ever seen). The adventure introduces an unlikely new contender for the Sea Lord's throne and takes the players into unexplored sections of the city, from the prison Hulks to the Temple of the Pirate God. It also creates a detailed mythology for Freeport's patron deity--and the cosmic villain who wants to shower unholy love on the City of Adventure.
