I play a monk who had such a good jump check I could often jump 30 feet in the air. My dm allowed me to take half of the 3d6 from the fall and add 3d6 to my attack. Eventually I took a skill trick so I didn't take any damage from the fall.

That's a DC of 120. 240 unless you get a 20ft "run". Clearly, that's epic level and thus the 3d6 I would say is inconsequential.

One combat benefit is that you get a +1 bonus to attack if doing so on "higher ground". I would grant such a bonus if you were standing on a table (jump check to get there).

Could Ninja levels count as Monk levels in order to get Mantis leap?

Also... what is maximum jump distance? I can't find anything on this.

Maximum jump distance is based on your check. It used to be limited by your height in 3.0, but that's gone.

Assuming you're landing right where you started, you wouldn't take damage from that anyway. Falling damage is based on the difference between your starting and ending heights, not the maximum height you achieve.

Can we get a book-quote for that? The SRD disagrees...
