Does it work the same or did they make major changes? I've been thinking of an ogre Fighter with LaiC and Monkey grip

The prerequisites are now that you're Large or bigger, with a natural reach of 10 feet or more. Beyond that, it's the same, except that the 3.5E version notes that a character that is successfully pushed back cannot move any farther in that round.
 

You know from a DM's point of view they've broken thet even more than it was.

Now no one will ever get into melee with that nasty fire breathing dragon with high dex and combat reflexes.

Cause they were just leaping into melee combat with all those weak ol' dragons that were lacking in the dex and combat reflexes departments. ;)

Remember that all dragons have a +0 dex mod and with an elite or rolled stat package they are really nasty and because of there nature ALL dragons should have at least an elite stat pak.

Even with a 10 that the parties strongest fighter that will never reach the dragon or any other huge+ creature again....nice feat but kinda stuffed i think.

And that's just an Adult's ability scores. He's already smarter than 95% of humanity... give us poor mortals a chance, eh?
 

All dragons should be unique that adult has the following:

+16
+00
+10
+06
+06
+06

as stated in the DMG. If your going to use dragons you have to make each one unique for the impact...that baby dragon that got away in the first advanture could come back with a vengance in later years....
