Something that they failed pretty hard on since Planescape is that the planes have no proven form - the Great Wheel is an observation based on the way portals and plane-crossing function, not an actual map of the cosmos. It was really annoying to me that they didn't point out that 4E didn't in any way remove the validity of the Great Wheel, and that they didn't put in the tiny handful of options required to make it entirely Planescape-compatible. It would take just a couple of pages.
